The clinical case
The difference between a surface scan and a full-contact pin scan isn't theoretical, it shows up in millimeters, and millimeters are what the orthotic is supposed to control for.
Captures the outside of the foot — what the skin looks like under no load. Soft tissue is compressible. The bony architecture underneath is what actually needs the support, but it's not what the camera sees.
Mill an orthotic off that scan and you often end up with a device that's 1–2mm too small across the arch and heel cup. Doesn't sound like much, until you remember the orthotic is supposed to control rearfoot motion to within fractions of a degree.
